And I'm guessing that your books or ledger show a cash balance of 25,135? Verify that first, and we can go from there.

    Balance per Bank.................... 23,208

    Add:
    o/s deposits............................. 5,640 (310 + 330 + 5,000)
    Less:
    o/s checks................................ <3,718> (90 + 50 + 3,578)

    Adjusted Bank Balance....................25,130

Yep, pk's got one side of the reconciliation done.

Now if I was correct in my assumption that your books are showing a cash balance of 25,135, just deduct that $5 service charge item from your balance, and your adjusted book balance is also 25,130, thereby matching the Adjusted Bank Balance that pk has come up with.
